How to apply on the bases of Demand Drafts photo copy
STEP 2: Click on

STEP 4: Click on marked area in snapshot

STEP 5: Download Application Form Format ( Page No. 2 & 3 )
STEP 6: Fill this Application Form by hand and do self attest i.e do Signature
at the last of page-2 with date and time.
STEP 7: Paste your photo on the first page of the Application Form.
STEP 8: Attach the Demand Draft Photo copy with this Application Form.
(The DD photo copy is through which you have applied earlier in a district)
STEP 9: Do the same for all 5 Demand Drafts Photo copy through which you have
applied earlier in 5 districts.
STEP 9: Do the same for all 5 Demand Drafts Photo copy through which you have
applied earlier in 5 districts.
STEP 10: Make a List of 5 districts for which you have applied earlier
and attach it with the Application Forms and Demand Drafts.
STEP 11: Dispatch these documents to the following address by Speed Post
so that it will reach by 31 Dec. 2012.

Note 1: If you want to apply more than five districts then you have to deposit
E-Challan of Rs. 500 for each district you want to apply.
